Role Type Changes the Clearance Bar
Not all jobs carry the same risk profile, and clearance criteria should reflect that:
- Safety-sensitive roles — driving for work, operating machinery, aviation, law enforcement, and similar occupations — warrant involvement from occupational medicine or physical medicine and rehabilitation (PM&R), since these roles typically require clearance beyond simple symptom resolution: reaction time, oculomotor function, and vestibular function should be reasonably normalized before full clearance.
- Cognitively demanding desk-based roles may benefit from a brief occupational therapy or neuropsychological cognitive screen before full unrestricted return, particularly when symptoms have persisted beyond roughly two to four weeks, to confirm the patient can sustain the cognitive load the job requires without symptom breakthrough.
- Physically demanding roles should be assessed for exertional symptom tolerance, similar in concept to graded exertion testing used in return-to-sport pathways, before clearance to full physical duty.
What a Graded RTW Plan Typically Includes
A written, individualized RTW plan generally addresses:
- Hours: a stepped increase (e.g., half-days before full days) rather than an immediate jump to a full schedule.
- Task restriction: temporary limits on multitasking, high-stakes decisions, or safety-critical tasks until tolerance is demonstrated.
- Breaks: scheduled rest periods to manage cognitive fatigue, which is one of the more common rate-limiting symptoms in the first two weeks.
